SmartCare Home Health

SmartCare Home Health is a home monitoring system that allows medical professionals access to health information, enabling them to make routine tests and health checks remotely.

Individuals can monitor a range of health conditions from home, the results are automatically directed to appropriate members of their care team. Should there be any unusual results, a member of the care team will contact the individual to discuss possible reasons why and should there be reasonable cause for concern, will direct the individual to a doctor, nurse, clinic or hospital. In many cases there is no need for alarm, anomolies may simply be due to user error, introduction of new medication or lifestyle changes.

The system permits the end user and health care team to form a patient specific 'trusted network'. Members of the 'network' can share information accross the secure SmartCare ExChange. The level of information any individual may access depends upon the level of security clearance they are given. Eg only medical staff will have access to full medical information but, by agreement, pharmacists and paramedics could have clearance for key medical information.

Individuals can undertake self-care programmes, where results are recorded Routine requests from or to the patient can be relayed automatically Advise can be given via the telephone or face-to-face via the optional video link, providing a 'virtual' clinic facilityPatients with medical needs who do not need hospital attention can be closely monitored at homeSocial Care and Health Care can be monitored over the same system ensuring a better overall 'view' of the person's needs.All calls are 'TRACKED' and all information including video is transferred to the appropriate case officer, doctor, nurse etc. A record is then held on the secure server making 'joijned up healthcare' a reality.The system can be used with a range of monitoring devices, and can be adapted to suit the patient's changing needs.
